The Bridge Parent Teachers Association (PTA) publishes a directory of class lists that contains student addresses, phone numbers, and parent information. The PTA also manages an email list (listserv) used for sending out school related emails.
Fill out this form ONLY if one of the following conditions applies to you:
1. Your child is an incoming kindergartener.
2. Your child is a brand new student to Bridge and was not listed in last year’s directory.
3. You need to make any changes to your Bridge directory/email list information from the previous year.
Instruction for filling out this form:
1. Fill out only the information you want published in the directory and sign where you are asked for your permission.
2. Sign the Announcement Email List permission at the bottom if you want email notifications from Bridge.
3. Return completed form no later than September 1st 2008 to Bridge PTA Directory, 55 Middleby Rd., Lexington, MA 02421.
Forms may also be dropped in the ‘Directory’ box in the Bridge front hall.
